Please be advised that we have established some dates for you to return all necessary paperwork for Football and Cheer.

The dates and times are as follows:

July 12, 13 & 14 6:00pm-8:00pm LOCATION: Tigers Fieldhouse

Each participant must have the following paperwork in order to participate in our program:

  • ORIGINAL BIRTH CERTIFICATE (new participants only)
  • Physical Fitness Form
  • Medical History Form (must be signed by a Dr)
  • Participant Contract
  • Parental Consent/Waiver
  • Tiger Cheer or Football Specific Signature Page
  • Final Report Card

PLEASE NOTE: YOUR CHILD WILL NOT BE PERMITTED TO PARTICIPATE IN OUR PROGRAM WITHOUT ALL OF THE MENTIONED PAPERWORK ABOVE. If you have any questions, or if those dates are not convenient for you, please feel free to call me anytime at 201-306-1713 or reply to this email.
